Cholmondeley accepts two types of referrals when applying for a child's stay: agency referral, or self-referral.
Please note that for the remainder of 2018, Cholmondeley will not be accepting referrals for first time child stays.
This is because Cholmondeley has experienced a significant increase in the demand for our respite care services. We recognise that to provide the best possible outcome for children within our care model, the children benefit from a series of short, quality stays with us throughout the year. The pause in accepting new children, for this short period of time, is so we can meet the respite care needs of our existing children by providing each child with a more optimal number of Cholmondeley stays specific to their circumstances.
Cholmondeley’s usual emergency response will remain available during this period so whether you are a main carer, community agency, school, or Oranga Tamariki, if you feel a child or family needs urgent support then please contact us and we will do our utmost to assist you.

Those referring a child to Cholmondeley who are from:
- a professional organisation or agency
- a school
- an NGO
- Oranga Tamariki
- a parent/carer
of the child/ren being referred, you may begin the referral process by downloading the self-referral form. You must print and complete the form by hand, before signing, and returning to us via one of the following methods:

Please note: A separate form must be complete for each child from one family. Please tell us as much as much about the child's situation as possible. All information you provide will be kept secure and only used for the purpose of assessing the referral and planning for children's wellbeing during their stay.
